Participant Flow Chart

  1. Individual hears about Colon Check.
  2. Individual calls Colon Check toll-free.
  3. Colon Check verifies eligibility.
  4. If an individual is Ineligible - they return to usual care.
  5. If an individual is Eligible - a FIT kit is mailed to their home*
  6. Kit is completed at home and returned to drop-off location.
  7. Colon Check analyses the sample.
  8. Lab sends family physician the FIT result.
  9. If the result is Normal, Colon Check mails result letter to participant. Participant is advised to re-screen in two years.
  10. If the result is Abnormal, Colon Check arranges for a Nurse Navigator to discuss follow-up options with the participant.
  11. If the participant is inappropriate for colonoscopy or declines - the family physician is notified.
  12. If the participant is appropriate for colonoscopy - the procedure is booked by the Nurse Navigator.
  13. Results and follow-up recommendations are sent to family physician.

*Anyone with two first degree relatives (e.g., parent, full sibling or child) or one first degree relative under age 60 - who have been diagnosed with colorectal cancer - will be referred directly to the Nurse Navigator for assessment for colonoscopy.
